STANDARD VISITATION/HOLIDAY SCHEDULE

I.      ROUTINE VISITATION: The non-custodial parent shall have Routine Visitation with the child(ren) every other weekend commencing on the ___ day of __________, 200_, from Friday at _____ o'clock ___.m. until the following Sunday at _____ o'clock ___. m.

II.     HOLIDAY VISITATION FOR NON-CUSTODIAL PARENT AND HOLIDAYS WITH CUSTODIAL PARENT:

        A.   In odd numbered years the non-custodial parent shall have the following holiday visitation:

        Memorial Day Weekend:    Friday at _____ o'clock ___.M. until Memorial Day at _____ o'clock ___.m.;

        Labor Day Weekend:          Friday at _____ o'clock ___.m. until Labor Day at _____ o'clock ___.m.;

        Christmas Vacation:           _____ o'clock ___.m. on the day the school district in which the child(ren) reside(s) lets out for Christmas Vacation until December 25th at _____ o'clock ___.m.;

        Children's Birthdays: _____ o'clock ___.m. until _____ o'clock ___.m.

        B.   In even numbered years the non-custodial parent shall have the following holidays:

        Easter Weekend:                Friday at _____ o'clock ___.m. until Easter Sunday at _____ o'clock ___.m.;

        July Fourth:                         July 3rd at _____ o'clock ___.m. until July 4th at _____ o'clock ___.m. unless the 4th is on a Monday or a Friday at which time visitation shall be if on a Monday from _____ o'clock ___.m. Friday preceding until _____ o'clock ___.m. Monday and if on a Friday from Thursday preceding at _____ o'clock ___.m. until Sunday at _____ o'clock ___.m.;

        Thanksgiving:                     Wednesday at _____ o'clock ___.m. until Sunday at _____ o'clock ___.m.;

        Christmas Vacation:           December 25th at _____ o'clock ___.m. until January 1st at _____ o'clock ___.m.

C.In odd numbered years the custodial parent shall have the following holidays: The same as the non-custodial parent had in even years.

D.In even numbered years the custodial parent shall have the following holidays: The same as the non-custodial parent had in odd numbered years.

E.Each year the Mother's Day weekend will be spent with the Mother and the Father's Day weekend will be spent with the Father.

F.The non-custodial parent shall have Spring Break of the child(ren) on odd years from the day school lets out in the school district in which the child(ren) reside(s) at _____ o'clock ___.m. until _____ o'clock ___.m. the day before school resumes.

G.The Holiday Visitation specified under Roman Numeral II (A-F) herein shall control over any Routine Visitation specified under Roman Numeral I herein and if the Holiday visitation supercedes an alternating routine weekend of the party normally having the child(ren), the alternating routine weekend will commence the next weekend after the Holiday Visitation weekend that so superceded and the parties will conduct the alternating weekends accordingly.

III.    SUMMER VISITATION: The non-custodial parent shall have a continuous summer visitation for a period not to exceed 5 weeks and the non-custodial parent it to provide the custodial parent with written notice of the requested continuous summer visitation at least 30 days in advance of the requested continuous summer visitation. The non-custodial parent shall allow one evening every two weeks for the custodial parent to visit with the child(ren) and allow reasonable telephone contact with the custodial parent to allow communication between the parents and child(ren).

 

IV.   COMMUNICATION: The parties are required to communicate with each other concerning the child(ren) of the parties and each party is to provide to the other any reports concerning the child(ren) of the parties progress or delays in school; medical, dental, or other health care necessities; health, dental, etc. insurance information; law enforcement contacts with the child(ren). The parties are required to allow the party not having the child(ren) of the parties in their home to have reasonable telephone and postal communication with the child(ren). In the event there is no prior written agreement to change permanent residence of the child(ren) or of the parent, either parent shall notify the other parent if such parent plans to change either of the permanent residence of the child(ren) or of the parent.

V.    This Order concerns minimal visitation and standards of conduct and the parties are required to provide more contact between the child(ren) and the non-custodial parent than specified herein. If the parties voluntarily change or alter any of the provisions of visitation or standards of conduct they are encouraged, if time and circumstances permit, to memorialize their agreements in writing by letters and notes.

VI.   This Order is incorporated by reference into the Decree of Divorce between the parents of the child(ren).
