[Ed. note: the form supplied by the Court's Bailiff in February 2003 was probably taken from a particular case and assigns custodian/visiting parent roles to the mother/father, respectively. This version has been modified to use the terms "custodial parent" and "non-custodial parent", not present in the original. While no "standard" visitation schedule exists, the bailiff advised that schedules with content like the above are typically used in Coal County. In the original format, the schedule appears to be text which is part of the main order and not a separate order. The "Transportation" paragraph probably needs adjustment. Adjust the form as needed.]
